The Aarogya Mitra is the primary contact for the beneficiaries at every Empanelled Health Care Providers

Entitled Beneficiary have to get himself verified onces in a lifetime through BIS Process laid down below.

On successful verification in BIS at PMJAY Counter the Entitled Beneficiary will be converted to "PMJAY Verified Beneficiary"

• Beneficiary have to submit the Aadhaar Card, Ration Card, PM Letter or any other Government ID Card.
• Aarogya Mitra will search name, verifies names based on Govt ID card, authenticate beneficiary, take a photo and establishes beneficiary relation with the family
• Aarogya Mitra will submit the record to Verifying Authority; once approved, the beneficiary is saved as "Golden Record-PMJAY Verified Beneficiary"
• Aarogya Mitra will then print the AB-PMJAY e-Card as “Golden Record” and give it to the beneficiary free of charge.

• Contact Aarogya Mitra at the Aayushman Bharat Counter of the Empanelled Hospital.
• Submit PM-JAY ID Card or Ration Card to Aarogya Mitra to Search Beneficiary and Register Patient.
